Appointments

Appointments, coaching sessions, consultations, and wellness programs must be scheduled through our approved booking methods.

Payments & Fees

ElleVive Women’s Health & Wellness currently operates on a self-pay (cash-pay) basis and is not accepting or billing insurance at this time.

Payment is due at the time services are rendered unless other arrangements have been made in advance.

Pricing, service fees, package costs, membership fees, laboratory fees, and other applicable charges are disclosed to patients prior to services being provided. Patients are encouraged to ask questions regarding fees, payment expectations, or available services before scheduling or treatment.

Accepted forms of payment may include major credit/debit cards, Health Savings Accounts (HSA), Flexible Spending Accounts (FSA), and other approved payment methods.

All pricing and fees are subject to change at the discretion of ElleVive Women’s Health & Wellness.

By scheduling appointments or receiving services, patients acknowledge and accept responsibility for all applicable charges and fees.

Cancellations & Rescheduling

We request at least 24 hours’ notice for cancellations or rescheduling requests.

Missed appointments or cancellations made with insufficient notice may result in:

  • Session forfeiture
  • Cancellation fees
  • Denial of refunds or credits

Refund Policy

Unless otherwise specified in writing, coaching services, digital products, wellness programs, and completed sessions are non-refundable.

Certain promotional offers, packages, or memberships may have separate refund terms disclosed at the time of purchase.


6. Protected Health Information (PHI) & Privacy

ElleVive Women’s Health & Wellness respects the confidentiality and privacy of your health-related information.

ElleVive Women’s Health & Wellness does not collect Protected Health Information (PHI) directly through this Website.
